Duly-completed Certification request forms are to be sent to the Spanish Society for Non Destructive Testing.

The data to be included in each form is:

• Method, Level and Sector for which the certification is sought
• Personal Data
• Current Job Position
• Academic education, other education or Postgraduate degrees
• NDT specialization courses
• Work experience
• Visual acuity and colour vision

List of Certificates and Documents to be submitted depending on each particular case (tick each submitted item in the corresponding list of the request form)

• Visual acuity certificate
• University Graduate certificate/Student record
• Training certificate, that needs to include the duration (total amount of hours)
• Work experience certificate, in months
• Fee Payment slipt
• Photocopy of the candidate’s Identity Card
• CV on NDT
• Other

If certification in more than one method is sought, a request form should be submitted for each method.
